Résumé

Le concept d'alexithymie de SIFNEOS et ses implications psychosomatiques sont comparés à une caractérologie psychanalytique, jugée comme trop rigide et limitée ; il est par ailleurs opposé aux apports les plus actuels de l'Ecole de Paris, à savoir essentiellement la contribution de MARTY dans une perspective économique nuancée et celle de McDOUGALL dans la perspective clinique de certains «anti-analysants».

Le travail original de KRYSTAL et sa description de «l'anhédonie» est ensuite envisagé dans ses rapports avec l'alexithymie et la nomination des affects par la mère, dans la perspective de l'utilisation des émotions nommées et reconnues comme signaux protecteurs pour le Moi.

Certaines considérations thérapeutiques enfin sont dégagées, à savoir le temps du «corps biologique» et le temps de la «nomination des affects» comme préalables à l'interprétation dans les déficits alexithymiques associés aux manifestations psychosomatiques les plus graves.

Samenvatting

Het begrip alexithymie van SIFNEOS en zijn weerslag op de psychosomatiek worden vergeleken met de opvatting waarin de psychosomatische stoornissen ondergebracht worden in een psychoanalytische karakterologie. Deze laatste opvatting wordt als te rigied en te beperkt ervaren. Deze zienswijze wordt ook tegengesproken, door de nieuwste inbreng van de Parijse school. Enerzijds is er de bijdrage van MARTY waarin hij een genuanceerd economisch standpunt inneemt ; anderzijds belicht McDOUGALL vanuit een klinisch perspectief op bepaalde patiënten, omschreven als «anti-analysandi», haar visie op psychosomatische stoornissen.

Verder behandelt de auteur de originele bijdrage van KRYSTAL en zijn beschrijving van de «anhedonie». Dit begrip wordt bestudeerd in zijn samenhang met de alexithymie en het benoemen der affekten door de moeder. Dit laatste wordt gesitueerd in het kader van de Ik-ontwikkeling : dank zij het feit dat de emoties kunnen herkend en benoemd worden, kunnen zij door dat zelfde Ik als beschermingssignalen aangewend worden.

Tenslotte worden bepaalde therapeutische gevoIgtrekkingen naar voor gebracht namelijk dat er een tijd bestaat voor «het lichaam in zijn bioIogisch aspect» en een tijd voor «het benoemen der affecten». Dit zijn de voorafgaande voorwaarden om te kunnen komen tot interpretaties in die gevalIen waar de tekorten der alexithymie gepaard gaan met zeer ernstige psychosomatische verschijnselen.

Summary

SIFNEOS' concept of aIexithymia and its psychosomatic implications are compared to psychoanaIytic characteroIogy and is reviewed, but considered as too rigid and limited ; alexithymia is opposed therefore to some of the most recent contributions of the Paris SchooI, namely MARTY's revised economic perspective and McDOUGALL's clinicaI description of the «antianalysant», as the fonctions in certain analyses.

KRYSTAL's original work and his description of «anhedonia» is then considered in its reIationships to aIexithymia and the naming of affects by the mother in the perspective, essential indeed, of the use of named and recognized emotions as protecting signaIs for the Ego.

Certain therapeutic consequences are stressed, namely the time of the «biological body» and the time of «affect naming» as prerequisites to interpretation in alexithymic defects associated with the more severe psychosomatic manifestations.
