Résumé

Cet article relate l'expérience d'un psychanalyste consultant dans un Centre de Traitement de la douleur ("Pain Clinic") et confronté à la douleur chronique psychogène.

Un tableau clinique en est fait, qui met l'accent sur l'identification projective et ses conséquences sur l'attitude contretransférentielle.

Deux exemples cliniques sont rapportés, l'un d'une psychothérapie. l'autre d'une psychoanalyse, de patients douloureux chroniques, dans le but de mettre en lumière quelques caractéristiques du fonctionnement mental de ces patients.

L'article passe en revue la littérature psychanalytique consacrée à la douleur.

La douleur est située dans ses relations avec la psychose et l'objet autistique, l'hystérie et la toxicomanie.

Elle est vue comme une pathologie à mi-chemin du psychique et du somatique, oscillant entre les défauts et les excès du refoulement.

In dit artikel beschrijft de auteur zijn ervaring als psychoanalyticus in een Centrum voor Pijnbehandeling ("Pain Clinic") waarbij geconfronteerd werd met psychogene chronische pijn.

Daarbij wordt een klinisch beeld beschreven waarbij de nadruk ligt op de projectieve identificatie en op haar gevolgen op de tegenoverdrachtelijke houding.

Om dit te illustreren worden twee klinische gevallen aangehaald. Het eerste betreft een psychotherapie, het tweede een psychoanalyse, telkens met een chronische pijnpatiënt. De auteur probeert daarin enkele karakteristieke eigenschappen van het mentaal funktioneren van deze patiënten te belichten.

De psychoanalystische litteratuur, gewijd aan pijn, komt eveneens aan bod. Verder wordt de pijn gesitueerd in zijn relaties met de psychose en het autistisch objekt, de hysterie en de toxicomanie.

De pijn wordt beschouwd als een pathologie die het midden houdt tussen een gebrek en een overmaat aan verdringing.

Summary

This article relates the experience of a psychoanalyst who is consultant to a pain clinic, where he is confronted with chronic psychogenic pain.

A clinical picture is drawn which accentuates projective identification and its consequences on countertransference attitudes.

Two clinical examples are given, one from the psychotherapy, the other from the psychoanalysis of patients with chronic pain in order to illuminate some characteristics of the mental functioning of these patients.

The article reviews the psychoanalytic litterature on pain.

Pain is placed in relationship with psychosis, the autistic object, hysteria and drug addiction.

Is is seen as a pathology midway between the psychic and the somatic, oscillating between the failures of and the excess of repression.
